Not everyone who is medically ready to leave hospital is immediately ready to return home. Pembroke Care provides the space, the environment and the flexible support for that intermediate period — a genuine step between the ward and your own front door.
Before arrival we liaise with the discharge team, so that medication, mobility and support needs follow you seamlessly. Most recovery stays run between two and six weeks: long enough to recover, rebuild confidence and regain independence; short enough to keep home firmly in view.
The pathway suits recovery after joint replacement and orthopaedic surgery, fracture recovery, reduced mobility following a hospital admission, deconditioning, and the loss of confidence that so often follows a fall or illness. Where wider clinical input is required, the pathway has been clinically and strategically developed with the expertise of Cloud Medical.
