The Hospital to Home pathway

One pathway home. Three ways to join it.

Post-Hospital Recovery, Transitional Living & Reablement, and Short-Term Respite & Recovery — one team, one environment, one aim: recover, rebuild confidence, regain independence, return home.

01

Post-Hospital Recovery

The step between hospital and home.

Short-term recovery support for people who are medically ready to leave hospital but not yet ready, confident or practically able to return home — after joint replacement, a fracture, a fall, or a hospital stay that took more strength than expected.

  • Referrals from discharge teams, professionals and families
  • Assessment in hospital or at home, typically within 24 hours
  • Planned and short-notice referrals considered
  • Typical stays of two to six weeks

02

Transitional Living & Reablement

Independence, rebuilt in a space of your own.

Time-limited support in a spacious, self-contained apartment — for people awaiting adaptations to their home, rebuilding daily-living skills, or needing a short period of support before returning to fully independent living.

  • Spacious, self-contained apartments in the centre of Reading
  • An assessed, flexible level of care and support
  • Person-centred reablement goals, reviewed with you
  • A bridge whilst home adaptations are completed

03

Short-Term Respite & Recovery

Planned breaks — and support when plans break down.

Planned respite stays and responsive support when a carer arrangement breaks down — the same environment, assessed care and recovery focus as every Pembroke pathway, from a single week.

  • Planned respite from a single week, based at Pembroke Lodge
  • Responsive cover when a carer arrangement breaks down
  • The same assessed care and support as every pathway
  • Full access to dining, gardens and shared spaces

Two settings, one pathway

Pembroke Lodge

Residential & respite care

Round-the-clock reassurance in the main house — for planned respite, and for guests whose assessment shows a residential setting suits them best.

Alexandra Lodge & the Pembroke Apartments

Transitional & independent living

Spacious, self-contained apartments — your own front door, with an assessed level of care and support arranged around you.

Every guest joins the same Hospital to Home pathway — assessment simply decides which setting, and which level of flexible support, is right.

Transitional care, and any personal care arranged through Pembroke, always come from our own CQC-registered service. For every other stay the choice is entirely yours: bring a care provider you already know, use ours, or stay without care and simply live independently.

Unsure which pathway is right?

Tell us about the discharge, the operation or the situation at home, and our care team will advise you candidly — including, on occasion, that a different setting would serve you better.
