From referral to returning home
A straightforward, carefully managed route from hospital — or from a home that is struggling — through recovery and back to independence.

Discuss a placement
One conversation with a senior member of our care team — about the discharge, the home that is waiting, and what a confident return would look like. Professionals and families are equally welcome to call.

Assessment, typically within 24 hours
We assess in hospital or at home, usually within a day of referral — and we tell you candidly if we cannot — so that medication, mobility and support needs are fully understood before arrival.

A planned arrival
Planned and short-notice stays are both considered. Your apartment is ready on the day, with the assessed level of care and support already arranged around you.

Live behind your own front door while strength and confidence rebuild — support that steps back as independence returns, and goals reviewed together every week.

We work to an agreed return-home plan with you, your family and the professionals around you — and we stay in touch once you are settled.
The environment
A different environment for recovery
Spacious, self-contained apartments in the centre of Reading — an environment considerably closer to normal independent living than a traditional institutional placement.
Assessed care and support
Personal care, medication support, mobility assistance and help with daily living — arranged at the level assessment shows is needed, and stepped back as independence returns.

A care team on site around the clock, with discreet call points in every apartment and night-time cover.
Structured recovery goals
Person-centred goals aligned with discharge, occupational therapy and reablement plans — agreed on arrival and reviewed as confidence returns.
Visiting clinics
GP visits arranged with local practices, plus visiting chiropody and hairdressing — recovery is never interrupted by errands.

Your own front door, living space and bathroom — considerably closer to normal independent living than a traditional institutional placement.
A space made your own
Photographs, a favourite throw, your own armchair — every apartment is yours to arrange for the length of your stay.
Transitional care, and any personal care arranged through Pembroke, always come from our own CQC-registered service. For every other stay the choice is entirely yours: bring a care provider you already know, use ours, or stay without care and simply live independently.

A warm sheltered environment staffed by dedicated carers, available 24 hours, was very reassuring and helped my recovery… The staff were very attentive to my needs and were professional and genuinely caring.
This was my dad’s first experience of care, on coming out of hospital… The staff were kind and helped him adapt… He left after two weeks much fitter, stronger, more mobile and more positive than when he arrived.
Mum was cared for very well. All the staff made her feel welcome and comfortable. Her food was lovely and she enjoyed the daily routine.

Everything you may wish to know
Yes. We take referrals from hospital discharge and transfer-of-care teams, social workers, brokerage, GPs, case managers and families alike. Call or email us and we will discuss the individual case the same day — informally, before any formal referral, if that is easier.
Typically within 24 hours of referral — and we tell you candidly if we cannot. A senior member of the care team assesses in hospital or at home, and both planned and short-notice placements are considered.
Most Hospital to Home recovery stays run between two and six weeks. Transitional living and respite stays are arranged entirely around the individual — from a single week to a few months whilst home adaptations are completed.
